We provide care to improve the quality of life for people who have an incurable illness. We care for patients from when they receive the terminal diagnosis to the end of their life.

Patients can receive our care at home, in nursing and care homes, and in the hospice.

What we do at Meadow House Hospice

We provide care for patients based on need and our aim is to care for the patient's:

  • medical symptoms
  • physical symptoms
  • psychological symptoms
  • family distress
  • social issues, or
  • spiritual challenges.

Inpatient care at Meadow House Hospice

Our 15 inpatient rooms have a fridge and a television for patient comfort. We have a large secluded garden, and a conservatory for patients to enjoy with their loved ones.

Patients receive care from a multi-disciplinary team and this may include::

  • physiotherapists
  • occupational therapists
  • psychologists
  • social workers
  • advocacy workers, and
  • chaplaincy.

A doctor will review the patients daily during the working week and out-of-hours if needed.

Visitors

Our visiting arrangements are flexible.

Loved ones can stay overnight after discussion and agreement with ward staff.

Visitors are welcome to use the sitting room which has a television, microwave, and facilities for making hot drinks.

Community palliative care team

Our community palliative care team help people with an incurable illness to receive care in the comfort of their home. The team includes:

  • community clinical nurse specialists (CNS)
  • doctors
  • physiotherapists
  • occupational therapists
  • psychologists
  • social workers
  • patient advocates, and
  • bereavement counsellors.

The community clinical nurse specialists provide specialist advice to manage difficult symptoms. They can help with other problems you or your family/carer may have, including:

  • pain relief and symptom control
  • link in with GPs to discuss changes in medication
  • listen to and working with you and your family or carers to address any concerns
  • support you and your family or carers during the last stages of illness
  • link in with hospital teams to ensure continuity of care
  • provide onward referral for help and support to other healthcare professionals in the team.

Lymphoedema service

This nurse led service provides advice and therapy for patients who have lymphoedema.

The treatment can include:

  • manual lymph drainage: focussed massage to move and reduce the fluid trapped in the tissues
  • bandaging
  • hosiery: specialised garments to maintain pressure on the limbs to prevent fluid returning.

Your GP, district nurse, specialist palliative care clinical nurse specialist or a hospital team can refer you to this service. We will then add you to the lymphoedema clinic on a waiting list basis. We will let you know when to attend the hospice for an assessment and planning session.

Psychosocial service

We are a team of clinical psychologists, social workers, chaplaincy and a patient advocate. We can help you cope with the emotional, psychological, social and financial difficulties that can develop when faced with an incurable illness.

You and your family can self-refer to this service or a member of our team can request a review for you.

For many patients and families when facing death and dying beliefs can be a source of strength. Our chaplaincy team can provide spiritual care and support for people of any or no faith.

Therapy service

The physical changes brought on by an incurable illness can create problems with mobility, self-care and general strength. We can provide:

  • help in home adaptations

  • extra equipment or walking aids, and

  • advice about diet and how to conserve energy.

We are not a dedicated rehabilitation service. We work with the local community-based services which includes:

  • physiotherapists who can assess mobility, strength and safety
  • occupational therapists who can provide help on practical matters such as dressing, bathing and how to cope with everyday tasks.

Your GP, specialist professional and other agencies can refer you to this service.

Bereavement service

Bereavement and the grief following a personal loss affects people in different ways. Whether it is expected or sudden, it can be overwhelming for some and bring about emotional and physical effects.

We can arrange one-to-one counselling with a professional with extensive counselling training.

How to make a referral to Meadow House Hospice

A healthcare professional can refer to our specialist palliative care team, if:

  • they have the patient's and/or carer's consent
  • the patient is registered with a GP in Ealing or Hounslow
  • the GP is made aware of the referral
