Community stroke project wins regional NHS award
A collaborative project delivering post-stroke therapy in community venues has been named a regional winner in the NHS Excellence Awards.
The LNWH Stroke Rehabilitation Programme partnered up with Chelsea FC Foundation and Watford FC Foundation to offer supervised physical activity, health coaching and peer support using the clubs' facilities.
The initiative, which has been funded by LNWH Charity, gives people recovering from stroke the confidence to rebuild their lives.
Figures showed 95% of the programme's participants felt more confident exercising by themselves, 85% were still exercising after three months, and weekly physical activity average averaged four hours.
Sophie Kempsford, Clinical Lead for Stroke, Neuro and Early Supported Discharge, said: “This programme shows what working in partnership can achieve with a programme co-designed with stroke survivors and co-delivered with community organisations. Many of the participants have described the programme as a 'lifeline'
The Excellence Awards celebrate the work of teams and projects making a difference to patients and communities reflecting the NHS's 10 Year Plan which is committed to renewing and rebuilding the health service. This includes improving health outcomes across the population.
There are now plans expand the programme across London and develop a integrated cardiac, renal and metabolic rehabilitation model using the same community approach.
