Patient safety incident response framework

The patient safety incident response framework policy and plan explain how we respond to patient safety incidents.

The framework helps us learn when things go wrong and improve patient safety for the future. It allows us to review and understand trends and themes in patient safety so we can make our systems and processes work better.

It’s based on four principles:

• Compassionate engagement and involvement
• System-based approaches to learning
• Considered and proportionate responses
• Supportive oversight.
