Other referral pathways

Some of our services and clinics accept referrals from non-GP referrers, teaching professionals, or parents or guardians. Some services accept paper referrals for clinically urgent cases only.

Stroke and TIA referrals

TIA referrals

Please do not use e-RS to refer patients who may have had a TIA.

If you think a patient may have had a TIA, please complete the TIA referral form.

GP referral form TIA[docx] 64KB

To complete the form, you must:

  • document the clinical history for your patient. This helps our consultants triage patients in order to see urgent patients first.
  • provide a list of your patient's current medication.

You can return the form to us by emailing our stroke TIA team.

You can also contact our TIA administrator on 020 8869 3214 or 020 8869 2298 (Monday to Friday, 8am to 4pm).

If you have queries or need advice, you can get in touch with our stroke specialist nurse by contacting Northwick Park Hospital switchboard or on bleep 640.

Acute inpatient referrals

If you need to discuss one of your patients with our specialist team, please contact the on call specialist doctor by contacting Northwick Park Hospital switchboard or on bleep 073.

You can get also in touch with our stroke specialist nurse by contacting Northwick Park Hospital switchboard or on bleep 640.

Outpatient referrals

We are happy to accept outpatient referrals from GPs, which should be submitted through the NHS e-Referrals Service.
